You will need to fill this out on a yearly basis. One birthday per day will be selected for our regular promotion, and one anniversary per month will be selected for our monthly promotion. Please submit entries two weeks in advance to be announced on air. Due to duplications, we would kindly suggest that you enter the middle initial of the birthday entry. Birthdays for twins need to be entered separately.
